His GP can use the consultations much more efficiently and specialists can make an important contribution with the use of one app. This way, everyone saves valuable time and David gets exactly the care he needs. Furthermore, David can digitally connect with other diabetics to share his experiences and learn from others. He can play an important role in his own care process.

Connection does not stop at healthcare for individual patients. In fact, it can mean so much more. Smart technology turns abstract loose data into valuable information. Through reports, GPs gain insight into the population and the healthcare provided. They can identify trends and developments in their patient portfolio, they can identify their sub-populations and create opportunities for valuable preventive care on a large scale.
For example, is there an excessively high number of vulnerable elderly people in a specific neighborhood? Then you as a GP can focus on this problem together with the district nurse and the elderly care specialist. In addition, a GP can could also look at the need for a hospice within this town or neighborhood.
